Postsecondary Credentialing Forms
Below are the forms required to be submitted when you are applying for a credential.
If you are applying for the first time, or if you are adding an Additional Pathway to your existing credential, please complete the “Credentialing Application - Postsecondary” and the “Occupational Experience” forms.
If you are holding an Initial credential and are moving to a Professional credential, please complete the renewal form.
If you are applying for a renewal of an existing application, please complete the “Renewal or Reinstatement Application - Postsecondary.”
If you are applying for a reinstatement of an expired credential, please complete the “Renewal or Reinstatement Application - Postsecondary” and the “Occupational Experience” forms.
